A night obs round's real skill is restraint, a torch beam kept low, to the foot of the bed rather than the face, so a check doesn't turn into a waking.

The whole point of this round is that the person in the bed doesn't notice it happened.
Why does the beam fall on the foot of the bed rather than on the patient's face?
Because light on a sleeping face is what wakes someone, and light on a blanket usually isn't.
A beam pointed at the foot still shows most of what a check needs — is the blanket moving with breath, has the position changed, is anything obviously wrong — without crossing a closed eyelid. Pointing the same torch a metre higher would answer the same questions and also end the patient's night, which would mean starting the round over with somebody now awake and unsettled instead of resting. The aim of the beam is doing real clinical work, not just being gentle for its own sake.
Why is the beam a soft ellipse rather than a hard, tightly focused circle?
Because a torch held at an angle, low and off to one side, throws light that spreads and softens by the time it lands — a hard, direct circle would mean the torch was aimed close to straight down and close to the sleeper.
The shape of the light is a side effect of the technique, not a design decision made after the fact. Hold a torch at the angle this round actually uses and a soft ellipse is what comes out. A crisp circle would mean someone was holding the torch wrong.
What is actually being checked for in that small pool of light?
Breathing, mostly — the rise and fall of the blanket, and how regular it is.
A hand can find a pulse and a monitor can log a number, but a few seconds of watching the blanket move tells a trained eye most of what it needs before either of those. It's a low-information check by design: enough to know nothing has changed, not enough to require waking anyone up to confirm it. Anything beyond that — an actual pulse count, a proper look — waits until there's a reason to want it, rather than happening as routine every single round.
Can a real ward, or a real patient, appear on a shirt like this?
Not a real patient — an actual person's face, or any detail that would make them identifiable, isn't something to reproduce regardless of who's asked.
The scene here has no face at all, which isn't just a compositional choice — it's the right one for the subject. A drawn bed, an invented blanket, a torch beam that belongs to nobody's actual night gets the whole idea of the round across without asking anyone to appear on it.
Does the same discipline, a small deliberate amount of light, turn up outside a hospital ward?
Yes, though usually aimed at the opposite goal.
A followspot's iris closes down to exactly the size of one performer and no further, which is the same control over how much light reaches a target, but built to be seen by a whole room rather than to go unnoticed by one sleeping person. A lighthouse's light is the reverse case again — built to be seen from as far away as possible. An obs round sits at the far end from both: the smallest, dimmest, most deliberately unnoticed use of a light in the dark.
Isn't checking on someone while they're asleep, without them knowing, a strange thing to be comfortable with?
No — the alternative is worse, and it's the one that actually disturbs sleep to deliver the same information.
Waking a patient every time a check is due, to ask how they're doing, would interrupt exactly the rest a ward exists to protect, and would also produce worse information — a person startled awake answers differently than one left to sleep. A check that gathers what it needs without a word exchanged is the considerate version, not the secretive one. The unnoticed round is the reason a patient can sleep through a night that's actually well supervised.
Does a beam of torchlight in the dark print well?
Yes, with the ellipse kept as a soft-edged glow rather than an even wash.
A true flat gradient across a large area tends to go patchy and grey on fabric; the working version builds the glow from a dense core fading to a few widely spaced flecks at the rim, which reads as soft from a normal viewing distance and prints as clean, separated ink at close range. Warm white carries the beam and the clipboard edge; the single pale green pinpoint for the monitor is the other mark on the shirt, and it earns its place by staying small.
